Distinguished Nurse of the Year Award

Application

OBJECTIVE:

This award is presented to a nurse who demonstrates excellence in caring for the oncology patient by recognizing a nurse that practices in a manner demonstrating excellence and compassion and who displays outstanding professional skills when caring for patients and families affected by cancer.

ELIGIBILITY:

A licensed registered nurse, chapter member and Oncology Certified preferred.

APPLICATION:

Includes three sections:

3 Letters of Support

SCORING:

Each completed application will be reviewed by the award committee. Answers to questions will be worth up to 15 points each. Letters of support are worth up to 10 points each.

AWARD:

An inscribed award and certificate will be presented at the Chapter's Annual Conference. The recipient of the award will also receive a one-year membership to the Oncology Nursing Society.
